Disability Support

The South Australian Government funds basic community support services for younger people living with a disability and their carers.

This funding is available to support younger people to live independently in their own home and community, assist in the improvement of their life, and prevent inappropriate or premature hospitalisation or institutionalisation.

Elgibility and Access

Younger people living with a disabling condition (under 50 years of age) are eligible to access support through SA HACC, this includes people living with a chronic illness that is disabling and which impacts on their activities of daily living.

People who are accessing support through the NDIS or other programs for similar supports are ineligible.

Access to the program is via a referral from a health professional, other service provider or self-referral (family referral). An assessment and eligibility review will be conducted by Pika Wiya Aged Care staff.
